Oh, you precious, wonderful person! Not joking -- secondary teachers often hear from students from the past but primary teachers seldom do. You can't imagine how much it would mean to her!
Contact the school board in the area where you went to school. Even if the school has closed, they will probably have records. If you explain why you are looking for her, I bet they'll help! I would start with the secretary at your former school... (they usually know everything!) The secretary can probably put you in touch with someone who knows your former teacher. Maybe another teacher who is still teaching at that school, etc.
Write out a nice letter to the person you want to honor and then see if you can either e-mail it to her or have a contact person deliver it to her. Include your information so she can get in touch with you. I'm sure she'll feel great hearing from you.
